Overview

Forms are how you collect the paperwork side of training: liability waivers, intake questionnaires, training agreements, and post-program surveys. You build a form from a template (or from scratch), send it to a client by email, and the completed response lands back in Pawgress.
Included on every planClient forms and intake are available on both Basic and Pro — no subscription required.

Finding Forms

Click Forms in the main navigation. It opens a menu with three sections:
  • My Forms — the forms you’ve created, ready to send
  • Submissions — every form you’ve sent and its status
  • Templates — the template library you build new forms from

Creating a Form from a Template

Step 1: Browse the template library

Open Forms → Templates. Templates are grouped into four categories: Use the category filter to narrow the list, and Load more to see additional templates.

Step 2: Fill in the placeholders

Templates contain placeholders — your business name, your cancellation window, and similar details that appear throughout the document. When you pick a template, Pawgress asks you to fill these in once, then substitutes them everywhere in the form. Some placeholders auto-populate from Pawgress data, such as the client’s name, the dog’s name, and your trainer name.

Step 3: Name and create

Give the form a name you’ll recognise later, then create it. The new form appears under My Forms.

Creating a Blank Form

If no template fits, open Forms → My Forms and use the create option to start a blank form. Give it a name and you’ll land in the editor.

Editing a Form

Open any form to edit it. In the editor you can:
  • Rename the form
  • Add, edit, and delete questions
  • Drag items to reorder them
  • Preview the form as your client will see it
Edits are batched — click Save Changes to apply them.
Changes to a form don’t alter responses that were already submitted. Past submissions keep the version of the form the client actually filled out.

Sending a Form to a Client

From the forms list

  1. Open Forms → My Forms
  2. Click the Send action on the form you want
  3. Choose the recipient — pick an existing client, or enter an email address to send directly to someone who isn’t a client yet
  4. Optionally select which dog the form is about
  5. Send
Your client receives an email with a link to the form. No Pawgress account is needed to fill it in.

From a client’s record

You can also start from the client side, which pre-fills the recipient for you — handy when you’re onboarding someone and working through their setup. Every form also has a shareable link you can copy from the editor and paste anywhere — a welcome email, a text message, your website. Responses submitted through a shared link are recorded as direct submissions.

Tracking Submissions

Open Forms → Submissions to see everything you’ve sent. Each row shows the client, the form, its category, and its status:
  • Sent — the client has been sent the form but hasn’t completed it
  • Completed — the client has submitted their response
Filter and search to find a specific submission, and open any completed one to read the full response.

What Your Clients See

Clients with a Pawgress account see a Forms page in their navigation, split into:
  • Pending Forms — forms waiting on them, with a button to fill each one out
  • Completed — forms they’ve already submitted, with the date they completed them
Clients who aren’t on Pawgress simply use the emailed link.

Tips

Build your intake pack onceMost trainers end up with a standard set: a waiver, an intake questionnaire, and a training agreement. Create all three from templates when you set up your account, then sending them to a new client takes seconds.
Send the survey at the end of a packageA short survey when an enrollment completes is the easiest way to collect testimonials while the client is still delighted with the results.
